Your sleep records, settings, and personal goals are stored in:

iCloud (optional): If you enable iCloud Sync as a Premium member, data syncs to your private iCloud account via Apple CloudKit — accessible only by you

DotSleep may request access to sleep data in the Apple Health app to automatically import sleep records.
Access is only requested with your explicit permission
Health data is never uploaded to any server
Data is used solely for display within the app
You can revoke access at any time in Settings → Privacy & Security → Health
